Do not allow an adult to move in without Home Forward approval. Please contact your Service Coordinator to add an adult to your household. They will help you get started with the process. Documentation may be required.
You must report the new child within 10 business days of the child’s birth, adoption, or court-awarded custody. Please contact your Service Coordinator. They will help you get started with the process. Documentation may be required.
Notify Home Forward within 10 business days if any family member will be away for 30 days or more.
When a household has no income, any new income for a household member must be reported within 10 business days of the change. Here are the steps to follow:
- To report changes in employment status, have the employer complete form "Verification of Employment Status".
- For any other new income, attach verification of the new income.
- To report changes in employment status, have the employer complete form "Verification of Employment Status".
- To report decreases in other types of income sources, such as benefits, submit documentation of the decreased income.
- If the adult reporting a decrease in income now has zero income, complete form "Statement of Zero Income".
- If you need another way to show the decrease, talk to your Service Coordinator.
A disabled family member may request to add a live-in aide to the household if a live-in aide is essential to their care and well-being.
Home Forward is required to provide reasonable accommodations in rules, policies, practices, or services to ensure a person with a disability has equal opportunity to use and enjoy our housing programs and services. This includes asking for a subsidy standard exception/extra bedroom for disability related reasons.
